Copy of valid passport or full travel document (all pages)

Proof of payment of the fee (Form 790 038) and proof of payment if not listed on Form 790 itself

A completed application signed by a remote worker (MIT Form)

Evidence of a minimum employment/professional relationship with foreign companies lasting at least three months at the time of application

Evidence of the actual and continuing activities of the foreign company or group of companies with which the employee maintains an employment or professional relationship for at least a year

ПCertificate of social security (if there is an agreement with the country of origin) or registration with Spanish social security, confirming at least the application for registration of a foreign company with social security and the obligation to register after obtaining permission before starting work

A valid certificate of the presence or absence of a criminal record in the last two years in the country or countries of residence. Additionally, a statement is provided that there are no criminal records in the last 5 years (this is not required if the person already has a residence permit or stay in Spain for more than 6 months and has already provided such information to obtain this permit)

Public or private health insurance. Travel insurance is not accepted (not required if registered with social insurance)
